It sounds like you are positioned to make someone you love feel bad. I can totally understand someone would have an idea of how it would go down in there head for a long time building up to it actually happening in their life. I would suggest just be very involved in the wedding planning and make that totally in line with what you want.
And if the relatives think it's “funny,” they're welcome to join Pennywise outside the wedding.
It will ultimately not hurt the bride in the long run if her family chooses to cleave to a literal clown over being supportive of her on her own wedding day. It will show her exactly who they are and where she sits on the clown hierarchy, and that kind of knowledge will ultimately empower her to move on from them.

Sensory issues shouldn't be that big of a deal. Go buy some wet wipes, and just say “go freshen up real quick and come right back.”
