   This dude is crappy and manipulative. The fact that he has been a good friend of yours doesn't mean that you can't do better or don't deserve better.

4. My therapist helps me a lot with seeing how things could change and get better, even when they look horrible. Maybe you could switch to weekly appointments.

   I hope things improve for you. This sounds like a very difficult situation, but one that could still get better with time and intentional efforts from you and her (she will need to work hard to figure out how to process her pain and her physical issues, and some of that will be outside your control).
