My husband is uncircumcised and I've never cared how his penis looks. I mean, no genitals are particularly pretty, but they get the job done lol. You are not responsible for keeping him alive. If he were to end his life, it would not be your fault.
If you do want to stay, not for fear of what he might do but because you want to be with him, it might be time for an ultimatum. Take your time, make notes and gather your thoughts and feelings- be ready for him to get defensive and angry but try to stay calm. If he cares and he’s invested in the marriage, you letting him know you cannot sustain the relationship and are unhappy enough to leave if he does not improve and actual work towards healing might be the wake up call he needs.
If he knows you’re unhappy enough to leave, and you’re seriously considering divorce, and still does nothing to help himself (and you) then that’s telling you all you need to do.
You’ve done all you can do, you have allowed him time to heal and get the help he needs but he has refused. If he continues that after knowing how close you are to leaving, then leave. You need to look after yourself. You cannot force him to get better if he doesn’t want to but that doesn’t mean you have to suffer with him.

Thanks again. It's been helpful to me to hear what others think, it has helped me to think things through and yes you're right. She believed me back when I told her I wanted an exclusive thing but she herself admits being insecure.
